A Continuum of Care That Meets Your Needs

Lutheran Social Services of Illinois (LSSI), a not-for-profit organization, works with seniors and their families to help them live as independently and safely in the community as possible. LSSI meets the changing needs of older adults in the Rockford area by offering a full continuum of services from home care to rehabilitation services. All current clients in any LSSI program have priority status in receiving services from other LSSI programs.

P.A. Peterson Center for Health
P.A. Peterson Center for Health is located in a beautiful, quiet neighborhood of northeast Rockford. That peaceful feeling is evident inside P.A. Peterson as well. Whether it’s apartment living (either independent or sheltered care), short-term rehabilitation, outpatient therapy, a community fitness program, nursing care services, special programs for people with Alzheimer’s disease/dementia, hospice care or respite care. P.A. Peterson serves individuals at all care levels.

In addition, P.A. Peterson offers one of the finest short-term rehabilitation units in Rockford. Private rooms, buffet dining, private telephone, personal television and state-of-the-art Nautilus equipment are just some of the “perks” our clients receive. We never lose sight of our client’s goal to return home quickly.

Intouch Home Care Services (Non-medical)
Intouch Home Care Services provides non-medical assistance for older adults in their homes. Trained home care assistants, including CNAs, help seniors with tasks such as housekeeping, laundering clothes, meal planning, minor home maintenance and grocery shopping. They also assist with personal care and provide medication reminders and transportation to appointments. Services benefit families by relieving the stress and worry associated with caregiving. Intouch Services are tailored to the unique needs of each individual.

In-Home Medication/Medical Management
Trained nurses work with client-specific medication profiles to help ensure that clients take medications in accordance with a physician’s orders. In addition, the nurses provide diabetic and blood pressure monitoring, as well as accompanying clients to appointments with their doctors. The benefits of these services include the prevention of medication errors, and the improvement of transitional care from hospital to home.

Legacy Corps
Legacy Corps provides volunteers who give respite support for caregivers of seniors and other loved ones who need ongoing attention. Legacy Corps members do not provide any medical or therapeutic services or tasks. They provide time off for caregivers to run errands, see to other responsibilities or just relax.
